Good Faith Estimate Notice

You have the right to know what your care will cost before you receive it.

Under the law, health care providers need to give patients who don't have insurance, or who are not using insurance, an estimate of the expected charges for medical services.

What you're entitled to

  • You have the right to receive a Good Faith Estimate of the expected cost of your treatment.
  • You can ask for a Good Faith Estimate before you schedule, at any time.
  • If you schedule a service, you'll receive the estimate in writing at least one business day beforehand.
  • If your final bill is $400 or more above your Good Faith Estimate, you may dispute the bill.
  • A dispute must be started within 120 days of the date on your bill.
  • Keep a copy of your Good Faith Estimate somewhere you can find it again.

Questions about an estimate?

Call our office at 818-850-2990 or get in touch. Asking about cost will never affect your care.
